Corail生物型股骨翻修柄联合钛缆捆扎在Vancouver B型股骨假体周围骨折翻修术中应用的效果分析

摘要(Abstract):

目的 观察Corail生物型股骨翻修柄联合钛缆捆扎在人工髋关节置换术后股骨假体周围骨折翻修术中应用的短期临床疗效。方法 回顾性分析自2019-03—2021-10手术治疗的18例人工髋关节置换术后Vancouver B型股骨假体周围骨折,术中采用钛缆捆扎固定骨折端,选用Corail生物型股骨翻修柄。初次双极人工股骨头置换的2例除翻修股骨柄外,增加髋臼侧置换。初次全髋关节置换的5例髋臼假体松动,2例因锥度不匹配而选择髋臼侧同时翻修,另外7例保留原髋臼假体。结果 18例均获得随访,随访时间6~36个月,平均20个月。1例术中出现股骨远端劈裂,加用1道钛缆捆扎。术后切口一期愈合,无感染、神经损伤、假体脱位及下肢深静脉血栓形成等并发症发生。术后有2例出现股骨柄下沉(≤3 mm),术后6个月随访时未再下沉,未出现假体松动迹象。随访期间患者无大腿及髋部疼痛,髋关节功能恢复良好,骨折均顺利愈合。骨折愈合时间为2.5~8.0个月,平均4.2个月。末次随访时髋关节功能Harris评分为81~92分,平均86.2分。结论 人工髋关节置换术后Vancouver B型股骨假体周围骨折翻修术中应用Corail生物型股骨翻修柄联合钛缆捆扎的短期疗效满意,手术操作相对简单,假体与股骨髓腔适配度高,可获得良好的股骨侧假体稳定,有利于骨折愈合并降低并发症发生风险。
